Alex Smith will miss his first start since 2010 tonight after he was unable to get medical clearance from a "neurologist" one week after sustaining a concussion, ESPN's Chris Mortensen reported. With Smith sidelined, second-year quarterback Colin Kaepernick will make his first career start against the Bears at Candlestick Park.
