He clearly needs surgery on both those ACLs, and he's playing at 80% so he doesn't pop them.

Shanahan has gotten into his head. It happens with all of his QBs
It's more due to the defenses all copying the Chiefs' approach. He's fallen back a bit because of it + the skill players being in/out of the lineup, while the OL has a fairly weak C/RT. He's still grinding it out and will need to eventually rise above it. He's still young and playing relatively well.
