Banks completely whiffs on the linebacker.
Even with Trent falling, he still takes his guy out of the play. If Banks makes that block, Mitchell is getting chunk yards.

Deebo's fumble really hurt our momentum. That first drive looked great until the fumble and no points there was crucial in the game.

in johnnydel's excellent breakdown, dwelley missed a tackle on LB smith that caused deebo to go more wide and safety to get a better angle to tackle him/cause the fumble. Ultimately, Deebo's fault but it goes to show you that little details make a big difference. Missing Kittle has a huge problem as no other TE is half the blocker or pass catcher he is. I was worried when I heard that he wasn't going to play and more for his impact in the blocking game. A healthy kittle makes that block on Smith and Deebo scores a TD as safety gets trucked by Deebo. Entire game then changes as bears become more and more hopeless.

(Being real here though, one of the interior lineman had to have misunderstood his blocking assignment, thinking Banks here)
