Trent Williams exited the game in the third quarter. The 49ers announced that the veteran offensive lineman sustained a groin injury and is listed as "questionable" to return. Jaylon Moore replaced him.
T Jaylon Moore (concussion, out)
Jaylon Moore, who replaced Williams, exited for the locker room in the fourth quarter. He was evaluated for a head injury. Colton McKivitz moved to left tackle, and Spencer Burford entered at right tackle.
The 49ers later announced that Moore suffered a concussion.
QB Brock Purdy (stinger, cleared)
Brock Purdy exited the game in the fourth quarter due to the same stinger he sustained in Week 15, per ESPN's Lisa Salters. While Purdy was cleared to return, the 49ers opted to keep Sam Darnold in the game.
G Aaron Banks (toe, questionable)
Aaron Banks exited late in the game. Ben Bartch replaced him. The 49ers announced that Banks is "questionable" to return due to a toe injury.
CB Ambry Thomas (hamstring, didn't return)
